Breast Cancer

  • Breast cancer occurs when cells in the breast grow abnormally and undergo unnatural multiplication.
  • GLOBOCAN 2018 Statistics show incidence of breast cancer to be 11.6 % of all new cancer cases and 25% of all cancers in women, their mortality has been claimed to be 6.6%.
  • Survival of breast cancer among US citizens with distant metastasis for 5 years is 36% – (Ref- Cancer Treatment Centre of America).
  • Annual screening is recommended by doctors, especially a mammogram, after age of 40, and patients are often found in early stage during such screening.
  • Types of Breast Cancer.
    – Ductal Carcinoma in Situ or DCIS.
    – Invasive Ductal Carcinoma.
    – Invasive Lobular Carcinoma.
    – Triple Negative Breast Cancer.

Breast cancer cases worldwide are linked to major lifestyle and other risk factors.

Signs & Symptoms

  • Lump in a breast.
  • Swelling in armpit.
  • Pain and tenderness in breast- lumps are mostly painless.
  • A change in a nipple- May be in the form of rash on or around nipple.
  • Change in breast size and shape, dimpling of skin of breast.
  • Unusual discharge from nipple which may be streaked with blood etc.

Physical Examination and Investigations

  • When a lump in breast is noticed, doctors use many tests to diagnose cancer.
  • It is ascertained if the cancer has spread or metastasized to other parts of the body.
  • Additional tests may also help the doctor decide the most effective treatments under the circumstances.
  • For most types of Cancer, a Biopsy is the only way to make a definitive diagnosis of cancer.
  • Imaging tests may be used to monitor the spread of cancer. For example : Mammogram, Breast Ultrasound, MRI etc.
  • Biopsy/ FNAC/Core needle biopsy.
  • Surgical (Open) biopsy.
  • Lymph node biopsy.
  • Biomarker investigations.
Further tests are done to study the cancer cells. These are : – A. Estrogen and progesterone receptor tests. B. Human epidermal growth factor type 2 receptor (HER2/neu) test – This has receptors that naturally bind with one of the two hormones estrogen and progesterone. C. Multigene tests like Oncotype DX and Mama print. Based on these tests breast cancer is described as –
  • Hormone receptor positive or negative (ER and or PR positive/ ER and or PR negative).
  • HER 2/neu positive or negative.
  • Triple negative – seen in multiple breast cancers that do not have any of the 3 receptors.
  • Triple positive – seen in multiple breast cancers that have all 3 receptors.

Chemotherapy

Radiation

  1. It is a treatment with high energy rays or particle that kills cancer cells also termed as External beam radiation.
