Information about Pancreatic Cancer

Pancreatic cancer is a disease in which cancerous cell growth occurs in the tissues of the Pancreas. The exocrine cells and endocrine cells of the pancreas have different type of function and it produces two different types of tumors. Exocrine pancreas cell produce digestive juices and endocrine pancreatic cells produce the hormones.

Pancreatic cancer has poor prognosis and recently its increasing incidence in India is alarming .

Pancreatic cancer is one of the cancer with worst prognosis. This disease is generally diagnoised at a very late stage when resection is not possiable. In such cases the median survival remains not more than few months. In spite of advances in chemotherapy there is hardly any improvement in increasing overall survival of patients with this type of cancer.

Our main approach in treating patients with pancreatic cancer remains to make the patient’s life comfortable by relieving his symptoms, improve his quality of life and increase his overall survival. Our therapies certainly increases the overall survival by several folds in these patients. Rasayana Therapy not only prevents but also treats the complications of Pancreatic cancer. Patient remains almost pain free and leads a good quality of life.

1. Symptoms of Progressive obstructive Jaundice – Tumors of the head of pancreas will cause dark urine, pale stools, skin itching.

2. Obstruction of common bile duct – It will cause dilatation of the gall bladder.

3. Purpura or bleeding -  Prolonged bile duct obstruction may cause abnormalities of coagulation, due to inadequate vitamin K–dependent clotting factors.

4. Steatorrhoea or malabsorption – (Steatorrhoea  is the presence of excess fat in feces. Stools may also float due to excess gas, have an oily appearance and can be especially foul-smelling)If the tumour causes extensive replacement of the pancreas, it may cause pancreatic insufficiency with, malabsorption and steatorrhoea and Diabetic Mellitus.

5. Pain in abdomen - Local invasion of tumor arising in body and tail of pancreas may give rise to severe pain.

6. Palpable mass - Tumor may be palpable as an epigastric mass due to local extension of tumor.

7. Weight loss and Cachexia.

1. Tobacco smoking.

2. Animal fats in diet.

3. Diabetes mellitus.

4. Chronic pancreatitis.

5. Surgery for peptic ulcer disease.

6. Chemicals- 2-napthylamine, DDT, Petrol derivatives.

1. CA 19-9 Tumor marker- The protein CA 19-9 is secreted by malignant cell from gastrointestinal tumors but very high levels are strongly suggestive of Pancreatic cancer.

2. ERCP- Definative diagnosis of carcinoma head of pancreas can be obtained by ERCP.

3. Biopsy - For confirmation of cancer and  type  of cells.

4. Percutaneous transhepatic cholangiography – It may demostarte dilated bile ducts.

5. Ultrasound or CT of abdomen – May demonstrate dilated bile ducts or pancreatic mass and guide fine needle aspiration cytology.

6. Chest X-ray , CT liver and abdomen, PET Scan  –to detect metastasis.

1. Surgery

. Radical surgery- After staging and investigations only 10% of patients will be considered suitable for definitive radical surgery called whipple surgery or pancreaticoduodenectomy.

Biliary bypass operation/hepaticojejunostomy- If the tumor is found to be inoperable at laparotomy, the surgeon can perform a biliary bypass operation to relieve the obstructive jaundice. This will typically involve joining the hepatic duct to the jejunum.

Gastroenterostomy- If there is duodenal obstruction due to large periampullary mass, then gastroenterostomy may performed.

2. Pancreatic enzyme supplements –It will help in malabsorption.

3. Vitamin k supplement- It is useful in treating coagulation disorders.

4. Chemotherapy- may have palliative role in patients.

5. Flurouracil and Gemcitabin - These have been licensed for this situation. However benefit is limited.
